Findings: According to the documentation, the facility failed to properly care plan appropriate interventions for the Alleged Victim’s (AV) exit seeking behaviors. On or about October 13, 2024, the AV exited the secured memory care facility by following a staff member out the door at approximately 2:00 pm. The AV was seen outside the locked facility walking on the sidewalk. A facility staff member was able to guide the AV back into the building, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
