Findings: The Alleged Victim (AV) relies on the facility to provide care and services. On or about October 11, 2024, the facility had replaced flooring in the hallway outside of AV's room, and the floor was not dry, and staff were instructed to not walk on the wet flooring. AV called for assistance multiple times throughout the night, but facility staff thought they could not walk on the wet flooring to get to AV's room. According to an investigation, AV experience pain and discomfort due to lack of care. The facility failed to provide care and services to AV, which is a violation of resident rights, and is neglect, which constitutes abuse. The Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) did not provide care and services to AV, which is a violation of resident rights, are considered neglect of care and constitutes abuse.
