Findings: The Alleged Victim (AV) relies on the facility for his/her care, has a history of urinary incontinence and has a catheter. On or about October 3, 2024, Alleged Perpetrator 2 (AP2) was notified that AV pulled his/her catheter out. AP2 took no action. Many hours later AV was complaining of lower back, abdominal, and genitalia pain and was transported to the hospital for care. AP2’s actions are considered neglect of care and constitutes abuse. It was also determined that the facility failed to ensure AP2 had been properly trained in all areas of their job duties, posing a serious risk of harm to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
