Findings: The Alleged Victim (AV) relies on the facility for his/her care. The Alleged Victim (AV) was care planned for assistance with transferring, toileting, and dressing. AV was known to have a sore on his/her buttock area. According to an investigation, AV was found soaked in urine multiple occasions and often left in his/her recliner for long periods of time without being repositioned, resulting in unreasonable discomfort, lack of dignity and placing AV at risk of serious harm. The facility failed to follow AV’s care plan, due to lack of staffing, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
