Findings: According to the documentation, the facility failed to provide a safe environment for the Alleged Victim (AV) by allowing them to exit the secured facility. On or about October 13, 2024, the AV was able to exit the facility by following someone out the front door. According to the AV’s service plan, they are known to try and exit the facility when others are leaving. The AV was found outside in the parking lot and was able to be redirected back into the facility. The facility failed to provide a safe environment by allowing the AV to exit the secured memory care facility, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
