Findings: The Alleged Victim (AV) relies on the facility for care, has a documented seizure disorder, and has a recent history of falls. According to an investigation, on or about July 13, 2024, AV had a fall, resulting in a 3 cm laceration above AV's right eye. The facility failed to provide services, to include implementing interventions to mitigate AV’s increasing fall risk and/or to ensure the facility was appropriately staffed, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
