Findings: The Alleged Victim (AV) has an unsteady gait and is a known fall risk with a history of falls. AV had a injury fall on or about August 1, 2024, and although was on alert, there were no interventions added to AVs care plan. According to an investigation, on or about September 11, 2024, AV fell and hit their head, requiring an emergency department visit and 17 stiches. The facility failed to provide appropriate services according to Alleged Victim’s needs, regarding care planning, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP25-00241 $1500.00 fine assessed
