Findings: On or about October 15, 2024, the facility fire alarm system was triggered, causing all memory care doors and gates to unlock. The Alleged Victim (AV) exited through one of the gates in the courtyard of the facility. AV was later found by staff in an unsecured area behind the assisted living portion of the building. AV did not suffer any injury during his/her elopement, however, AV was placed at risk of serious harm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
