Findings: Alleged Victim (AV) is a resident of the Resident Care Facility Memory Care Unit and relies on the facility for his or her safety. AV is known wander within the facility and outside. AV is reported to take walks in the afternoons. On or about October 15, 2024, AV was outside in the courtyard unsupervised and wondered through an open, unlocked courtyard gate. It is believed the back gate was left open by the landscaping company. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-01045 $375.00 fine assessed
