Findings: The Alleged Victim (AV) moved into the facility in July 2024. AV was a known fall risk and had a previous fall at home where AV suffered a fractured hip. AV suffered approximately 5 falls between August 18, 2024 and October 17, 2024. On or about August 28, 2024, AV suffered a fall in which AV hit his/her head and was bleeding. AV was sent out to the hospital and was diagnosed with a fractured hip requiring surgery. The facility's failure to properly care plan for AV's risk of falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-01257 $1500.00 fine assessed
