Findings: The Alleged Victim (AV) relies on the facility for their care. According to an investigation, on or about August 21, 2024, the AV reported the Alleged Perpetrator 2 (AP2) had been handling the AV in a rough manner when providing care, causing the AV fear on multiple occasions, which resulted in the AV experiencing unreasonable discomfort and loss of personal dignity. The facility failed to complete a thorough investigation regarding the allegations of the AP2’s rough treatment of the AV. The AP2’s actions are a violation of resident rights, are considered neglect of care and constitutes physical abuse. The facility failed to protect the AV from physical abuse causing emotional harm.
