Findings: On or about a time prior to October 22, 2024, Alleged Perpetrator #2 (AP2) was called into the Alleged Victim's (AV) room to look at a wound by Witness #2 (W2) and Witness #3 (W3) while they were cleaning AV up after a bowel movement. AP2 entered AV's bathroom and called AV "stinky" and "disgusting" due to the feces in AV's room. AP2's actions are a violation of resident rights, are considered neglect of care and constitute verbal abuse. The facility's failure to protect AV from verbal abuse is a violation of Oregon Administrative Rules.
