Findings: The Alleged Victim (AV) relies on the facility to manage his/her medications. The AV is care planned to have AV a meal within thirty minutes after receiving insulin. According to an investigation, on or about October 22, 2024, AV was not able to eat before getting on a bus for a medical appointment, causing AV to experience low blood sugar, placing AV at serious risk of harm. The facility failed to provide a safe medication administration system and failed to administer medication as ordered, which is a violation of resident rights, is neglect of care and constitutes abuse. The allegation that Alleged Perpetrator 2 (AP2) neglected the AV was investigated and determined to be not substantiated.
