Findings: On or about October 23, 2024, Alleged Victim (AV) went into the courtyard, eloped by climbing over the fence and was found and returned to the facility by Local Law Enforcement. AV was not injured or harmed in the course of AV's elopement. However, at time of incident AV's care plan was not being followed which resulted in AV leaving the facility without assistance and exposing AV to potential harm. The facility's failure to follow AV's care plan due to AV's elopement risk is a violation of Oregon Administrative Rules.
