Findings: Alleged Victim’s (AV’s) care plan notes AV cannot be in his room alone at any time due to know history of falls. AV sustained falls on or about September 15th and 20th, 2024, October 23rd, 24th, 26th, 27th, and 29th 2024. During the falls which occurred on October 2024, progress notes indicate that in each of those falls AV was “found by staff” which may indicate AV may have been left alone which is a violation of the care plan requiring that AV not be left alone, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP26-00057 $375.00 fine assessed
