Findings: On or about August 2024, facility management determined that the Alleged Victim (AV) required a higher level of care than the facility could provide. AV's service plan still stated that AV was independent in dressing, toileting, transfers and mobility. AV experienced at least 6 falls between September 9, 2024, and October 28, 2024. The facility did not implement any changes to AV's care plan or put into place any interventions to reduce the number of falls that AV was experiencing. The facility failed to properly care plan after AV experienced a significant change of condition, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP25-00224 $250.00 fine assessed
