Findings: On or about October 26, 2024, the Alleged Victim (AV) was returning from the facility after being out. AV was walking back to the facility where there was an extension cord on the ground, crossing the walk way. AV tripped on the extension cord, fell and had a bloody nose. AV was transported to the hospital where he/she was diagnosed with a fractured nose. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP24-01116 $500.00 fine assessed
