Findings: Witness #2 (W2) has a history of being verbally aggressive and not complying with the Alleged Victim's (AV) care plan. AV is care planned to have visits with W2 in the common area only. On or about October 30, 2024, W2 and AV were found in AV's room alone. Staff walked into AV's room and found W2 on top of AV, and got up and slapped AV on the lower extremity of AV. The facility's failure to follow the care plan placed AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-00094 $188.00 fine assessed
