Findings: The Alleged Victim (AV) relies on the facility for his/her care, has a history of seizures, and is a known fall risk. According to an investigation, AV is a total assist for seizure disorder and confusion, and AVs Service Plan states most of AV’s falls are seized related. Between September 25, 2024, and October 30, 2024, AV is documented to have gone to the hospital approximately three (3) times for a fall or a seizure that resulted in injury to include head hematomas and lacerations. The facility failed to implement interventions or appropriately care plan, to mitigate the risk of injury due to AV's increase in seizures and falls, which is a violation of resident rights, is neglect of care which constitutes abuse.
