Findings: The Alleged Victim (AV) was care planned to have a pool noodle under his/her sheets to prevent him/her from falling from his/her bed. AV was also to have a fall mat in place in case AV fell from bed. On or about October 27, 2024, AV was found on the floor with his/her head against the wall. Emergency services were called to have AV evaluated. AV suffered a skin tear to his/her knee. AV's pool noodle was not on his/her bed and his/her fall mat was not in place. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP25-00123 $250.00 fine assessed
