Findings: The Alleged Victim (AV) relies on the facility to provide care. According to an investigation, on or about September 9, 2024, AV pressed the call light and it was not answered for approximately twenty-nine minutes, due to lack of staffing, posing a serious risk of harm. The facility failed to provide a safe environment by failing to answer AV's call light timely, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
