- The Alleged Victim (AV) relies on the facility for his/her care. AV is known to wander and has a history of exit seeking behavior. According to an investigation, on or about November 3, 2025, AV eloped and was outside for approximately thirty minutes, posing a risk of harm. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
