Findings: The Alleged Victim (AV) relies on the facility for medication management. According to an investigation, Between October 31, 2024, and November 3, 2024, AV received a higher dose of his/her medication on multiple occasions, resulting in AV feeling "cloudy and slow" and not able to think straight. The facility failed to provide a safe medication administration system, which is considered neglect of care and constitutes abuse.
