Findings: Both the Alleged Victim (AV) and Witness 1 (W1) rely on the facility for care. W1 has a history of aggressive behaviors resulting in resident-to-resident altercations and is care planned for staff to keep an eye on W1 when around other residents. On or about November 4, 2024, W1 came out of their room and pushed AV down, resulting in AV hurting their back and knees. The facility failed to implement interventions to address W1 aggressive behaviors and provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
