Findings: The Alleged Victim (AV) relies on the facility for their care. The AV has a known history of bruising and injuries of unknown origin. According to an investigation, on or about November 6, 2024, the AV was identified with a bruise to the right inner forearm. Three additional bruises were identified on AV's right elbow and posterior bi-cep. The facility failed to implement interventions according to the AV’s needs to mitigate injuries, which is a violation of resident rights, is neglect of care, and constitutes abuse.
