Findings: The Alleged Victim (AV) is a known fall risk and had suffered falls three times in the month of October 2024. On or about November 7, 2024, AV was found on the floor next to his/her bed, with his/her blanket and pillow and blood covering AV. AV was sent to the hospital immediately and diagnosed with a fractured nasal bone, bruising and multiple skin tears. The facility's failure to implement interventions to prevent AV from falling is a violation of resident rights, is considered neglect of care and constitutes abuse.
