Findings: The Alleged Victim (AV) is a known fall risk and suffered 3 separate falls between November 3, 2024 and November 8, 2024. On or about November 8, 2024, AV was found on the floor after a fall out of his/her bed. AV was taken to the hospital and diagnosed with a fractured femur from falling out of bed. On or about November 4, 2024, interventions for falls occurring on November 3rd and 4th were to change AV to a hospital bed, to reduce AV's falls. On November 8, 2024, at the time of AV's fall, AV was not in a hospital bed. The facility's failure to ensure interventions were appropriate and in place is a violation of resident rights, are considered neglect of care and constitute abuse.
