Findings: The Alleged Victim (AV) had wandering behaviors and a history of elopement. According to an investigation, on or about November 24, 2024, the AV eloped from the facility for the second time in less than a month, placing AV at risk of harm. At approximately 8:20 pm, it was discovered that AV eloped from the facility when another resident alerted staff that the AV was in the back parking lot of the facility, and the AV knocked on the window of another resident. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
