Findings: Alleged Victim (AV) relies on the facility for his/her care and has a history of falls. On or about November 10, 2024, AV had an unwitnessed fall where AV hit their head, resulting in head and cheek pain and AV went to the hospital for treatment. According to an investigation, AV had a previous fall on November 4, 2024, resulting in a bruised arm, and there were no additional interventions put in place to prevent further falls. There were also additional injury falls after November 10, 2024, and again, no additional interventions were added to AV's service plan. The facility failed to provide services, to include implementing additional interventions after a fall and provide a safe environment for AV, which is a violation of resident’s rights is neglect of care and constitutes abuse.
