Findings: The Alleged Victim (AV) is a known fall risk and had suffered multiple falls between September 7, 2024 and October 18, 2024. Interventions put in place appeared to be ineffective, as AV continued to have falls. AV was care planned for frequent safety checks and reminders to use his/her walker when ambulating. On or about November 10, 2024, AV was found on the floor when staff went to complete safety checks. On or about November 11, 2024, AV was again found on the floor from a fall when staff were arriving to do safety checks. The facility's failure to ensure adequate oversight of AV, leading to continued falls, is a violation of resident rights, is considered neglect of care and constitutes abuse.
