Findings: The Alleged Victim (AV) has a known risk with a history of falls and identified decline in condition. According to an investigation, the AV experienced multiple unwitnessed falls between approximately September 20, 2024, and October 30, 2024, resulting in pain, injuries, and repeated discomfort. On or about November 15, 2024, the AV was found on the floor with abrasions and bruising to the head and signs of discomfort. The facility failed to implement interventions according to the AV's needs relating to their decline in condition and number of falls. The failure is a violation of resident rights, is considered neglect of care, and constitutes abuse.
