Findings: On or about November 16, 2024, it was discovered by staff that the Alleged Victim (AV) was missing medication. AV's medication was transferred to a new pharmacy. When AV's medication arrived, another staff member was not aware of the change and sent AV's medication back to the pharmacy, resulting in AV missing 11 doses of his/her medication, which caused AV to be shaky, antsy and feeling ill. The facility's failure to provide a safe medication administration system is a violation of resident rights, is considered neglect of care and constitutes abuse.
