Findings: The Alleged Victim (AV) relies on the facility for care and is a known fall risk, with an extensive recent history of falls. AV is care planned for staff to check on AV eight times per shift for safety due to fall risk and wandering requiring redirection. According to an investigation, on or about November 16, 2024, AV fell and was on the floor in pain. The last documented time staff checked on AV was approximately 13 hours earlier. The facility failed to provide appropriate services according to Alleged Victim’s needs, regarding implementation of services, safety checks and monitoring, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
