Findings: The Alleged Victim (AV) relies on the facility for assistance with transfers and toileting. The AVs service plan directs staff to provide full assistance with all transfers and partial assistance with toileting. According to an investigation, facility staff refused to assist the AV with transfers, resulting in incontinence episodes and a loss of dignity. The facility failed to provide care as service planned, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
