Findings: On or about November 20, 2024, Witness #1 (W1) was witnessed rubbing the Alleged Victim's (AV) breast while sitting next to each other at the breakfast table. W1 has a known and documented history of sexual behaviors. W1's care plan failed to have specific interventions regarding W1's history of sexual behaviors. The facility's failure to protect AV from sexual abuse is a violation of resident rights, is considered neglect of care and constitutes abuse.
