Findings: Alleged Victim (AV) is care planned to receive skin cheeks during care. On or about November 21, 2024, AV was discovered to have a skin tear of unknown origin on his/her left forearm. Although AV is service planned for skin checks, the facility has not place interventions in place to prevent skin tears on AV, only to monitor skin for new skin issues. The facility failed to care plan around AV’s skin care, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP25-00419 $500.00 fine assessed
