Findings: The Alleged Victim (AV) relies on the facility for his/her care, and is care planned for assistance with transfers. On or about August 4, 2024, the Alleged Perpetrator 2 (AP2) was attempting to transfer AV, when AV and AP2 fell to the floor, resulting in AV sustaining a back fracture. According to an investigation, AP2 was a new caregiver and was not following AV's care plan which resulted in the fall. AP2 was not fully trained prior to the facility scheduling AP2 to work alone. The facility failed to provide services, to include ensuring care plans were followed, qualified staff were present, and provide a safe environment for AV, which is a violation of resident’s rights is neglect of care and constitutes abuse.
