Findings: According to the documentation, the facility staff failed to follow the care plan for the Alleged Victim’s (AV) known history of exit seeking behavior. AV’s service plan states the staff are to keep eyes on the AV to ensure they do not follow visitors out the door. On or about September 17, 2024, the AV followed a visitor outside of the locked memory care unit and into another part of the campus building. The staff were unaware the AV had exited the secured memory care unit. Staff from another unit notified the memory care a resident had gotten out of the secured unit. The AV was redirected back into the secure unit. The failure to follow the care plan to ensure the AV did not follow a visitor out of the secured unit placed the AV at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
