Findings: Alleged Victim (AV) has a known history of falls. On or about November 30, 2024, staff saw AV on the shower floor laying on his/her side with his/her head against the shower wall. No walker was in sight or close to AV. AV did not have his/her non-skid socks on or glasses on his/her face. Blood was noticed coming from AV’s head. AV complained his/her head hurt and also his/her hip and right shoulder. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP25-00655 $500.00 fine assessed
