Findings: The Alleged Victim (AV) is a resident of the facility and relies on facility staff to meet AV’s daily care needs. AV is protective of their space. Witness 1 (W1) is known to wander into other resident’s rooms, have aggressive behaviors, and has had multiple altercations with the AV in the AV’s room. According to an investigation, on or about, November 21, 2024, W1 entered the AV’s room and engaged in an altercation with the AV, resulting in unreasonable discomfort. W1’s service plan lacked interventions to address W1’s wandering behaviors to ensure a safe environment for the AV, which is a violation of resident rights, is considered neglect of care, and constitutes abuse.
