Findings: The Alleged Victim (AV) relies on the facility for his/her care and is a known fall risk, partially due to an increase in medications prescribed to manage AVs behaviors. According to an investigation, between October 12, 2024, and December 10, 2024, AV had approximately 6 falls, some resulting in injury including but not limited to a head injury, skin tears, abrasions and bruising. The facility failed to provide services, to include implementing effective interventions to mitigate AV’s falls and monitoring AV for safety, resulting in repeated unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
