Findings: The Alleged Victim (AV) is dependent on the facility to meet his/her safety needs and has a history of altercations with other residents. According to an investigation, on or about December 4, 2024, AV was involved in a physical altercation which resulted in AV getting slapped. The facility failed to develop interventions to address AV's behaviors. The facilities failure to properly plan care resulted in harm, which is a violation of resident rights, is considered neglect of care which constitutes abuse.
