Findings: Witness #1 (W1) is known to wander. On or about December 4, 2024, W1 wandered into the Alleged Victim's (AV) room. AV found W1 standing over his/her spouse and asked W1 to leave. W1 and AV engaged in a physical altercation where AV received a minor laceration and a skin tear to his/her face. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
