Findings
Alleged Victim (AV) is known to exit seek. Staff are to redirect with offering 1:1 time to AV, per AV’s care plan. On or about December 4, 2024, Witness 1 (W1) received a phone call from Witness 2 (W2) at local facility. W2 called to inform W1 that AV was outside across the street of facility at a local business. W2 called to inform W1 that W2 was getting AV and bringing AV back to facility. W2 and W1 then assisted AV back into the facility. W1 then helped AV take off AV's jacket and assessed AV for any injuries. AV had scattered bluish/purplish discoloration on AV's left forearm along with a skin tear. Two separate blackish, and purplish marks on AV's left forearm, one of discoloration marks was large about 4-inches in length, and other was about the size of a quarter. AV was found with rip in skin and larger discoloration mark. Skin tear about the size of a fifty-cent piece. Three strip stitching covering skin-tear on AV's left forearm. AV was previous seen 20-25 minutes before leaving the facility. Alleged Perpetrator 2 (AP2) had gone out the front door, and the front door was shut, unfortunately there is about a 15-second lag on doors locking. AP2 did not hear or see AV come out of facility door behind staff member and AP2 did not confirm the door was secured before walking away from the door. AP2 failed to ensure that facility door locked when leaving, which resulted in AV eloping outside of facility and receiving a skin tear, which is neglect of care and constitutes abuse. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
