Findings: The Alleged Victim (AV) has a history of wandering into other residents rooms. AV has been involved in multiple altercations due to wandering into others rooms. On or about December 8, 2024, AV walked into Witness #1's (W1) room and W1 led AV out of his/her room. W1 held onto AV's wrists, stomped on AV's foot and punched AV before being separated by staff. The facility's failure to ensure interventions were appropriate to reduce the risk of altercations is a violation of resident rights, is considered neglect of care and constitutes abuse.
