Findings: The Alleged Victim (AV) and Witness #1 (W1) have a history of altercations. Both residents have interventions in place, AV is to be redirected when wandering and W1 is to be redirected when agitated. On or about December 8, 2024, AV was wandering and W1 was in the common area. AV was taking items, which irritated W1, leading W1 to walk to AV and try to take away the item that AV picked up. W1 then hit AV in the arm. Facility staff failed to redirect either resident. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
