Findings: The Alleged Victim (AV) is a known fall risk, as AV's family advised the facility upon move in that AV had suffered falls at his/her previous placement. AV suffered three falls over 4 days. AV was sent to the hospital a few times after the falls. AV did not have a fall mat, bed alarm or chair alarm to alert staff of falls. AV is one person assist with transfers and one person assist with moving through the cottage in his/her wheelchair. The facility's failure to ensure AV's safety while ambulating is a violation of resident rights, is considered neglect of care and constitutes abuse.
