Findings: The Alleged Victim (AV) relies on the facility for care and is not documented as a fall risk despite having approximately six (6) documented falls between October 2, 2024, and December 10, 2024. According to an investigation, on or about December 11, 2024, AV was found on the floor after a fall, resulting in bruises. The facility failed to provide services, to include implement effective interventions to mitigate AV’s falls and monitoring AV for safety, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
