Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV is at risk for elopement and has recently been experiencing a condition affecting muscle control, making AV a fall risk. According to an investigation, on or about November 11, 2024, AV fell, resulting in a head injury and visit to the hospital. On or about December 12, 2024, AV was picked up by an outside transportation facility for a medical appointment. Facility staff did not escort AV to his/her appointment and was dropped off at a different clinic location than where AV’s appointment was scheduled, posing a risk of serious harm. AV wandered around by AV’s self for approximately fifteen minutes before medical personnel at the clinic assisted AV. The facility failed to provide appropriate services according to resident needs, which is a violation of resident rights, is neglect of care which constitutes abuse.
