Findings: The Alleged Victim (AV) and Witness #1 (W1) are known to engage in altercations. AV requires frequent checks and redirection while wandering in the facility. W1 has episodes of agitation and is triggered by loud noises. AV and W1 are to be kept apart as much as possible. On or about December 12, 2024, AV was found on the floor with a chair flipped over next to him/her. W1 was yelling at AV to get up and was raising his/her leg towards AV as if to kick at AV. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
